Dear OLSH Community,

 
Our Lady of the Sacred Heart is in the midst of a self-assessment process that includes feedback and input from the full school community including students, their families, teachers, and other members of the larger OLSH community. As such, we are conducting a survey to evaluate overall satisfaction with the school.
 
Your participation in this survey is anonymous, and the data collected will be used by a committee at OLSH to determine the strengths of our school, as well as the areas that may need improvement. The results of the survey will also be provided to a team of visitors that will visit our school to validate the self-assessment we have completed and determine if OLSH should be re-accredited by the Middle States Association Commissions on Elementary and Secondary Schools.
 
The following survey should take about 10 minutes to complete. There are  three open-ended questions at the end of the survey where you are encouraged to share written comments. None of the survey questions are required, but please feel free to answer as many as pertain to your experience.
