Through your Blackbaud account, you should have received a notification of 1 Checklist (which includes 7 individual forms) that MUST be completed by August 26, 2022. Once you have logged into your Blackbaud account, the forms are located in the yellow bar at the top of the screen.

Below are the forms that need to be completed by 8/26.

Parent Forms:

  • Profile update: Form to confirm your demographic information that was imported from PowerSchool
  • Emergency Contact: This form provides emergency contact information for your student(s) in the event a parent cannot be reached. Emergency contacts were not imported from PowerSchool, so we ask that everyone update this section.
  • Medical Update: This medical form provides OLSH with information regarding medical issues and needs such as allergies and other medical diagnoses that OLSH administrators should know in the event there is an emergency on campus and a student needs medical assistance.
  • Media Release: This is a permission form allowing OLSH to utilize your student's photo in school publications.
  • Parent-Student Handbook: By signing this form, you are acknowledging that you have read, understand, and agree to the policies outlined in the Parent-Student Handbook. Link to the handbook is on the Checklist, but approval is a School Form - please access from the checklist.
  • Responsible User Agreement: By signing this form, you are acknowledging that you have read, understand and agree to the policies outlined in the Responsible Use Agreement regarding the internet and using technology responsibly.
  • Chromebook Agreement: By signing this form, you are acknowledging that you have read, understand and agree to the policies outlined regarding expectations for your student’s use of the school-issued Chromebooks. Link to the Chromebook agreement is on the Checklist, but approval is a School Form - please access from the checklist.
